On 11/16/23 the whereabouts of a 5 year old child in care were unknown to the provider however the information was not reported to the department as required. The department learned of the incident on 11/20/23 when 2 complaints regarding the incident were received by the department. |

The center administrator failed to provide the parents of an enrolled child with sufficient detail to reasonably apprise the parent of an incident involving the child's whereabouts being unknown to the provider when the parent was informed that Staff A sent their 5 year old outside to the outdoor play area where the child "couldn't find her group." What actually occurred, per video footage that was available at the time of the incident by not reviewed by the center is Staff A fails to notice the child leave the group & enter an empty room eventually exiting the building & becoming locked out. |

Each child was not supervised by a child care worker who is within the sight and sound of the children to guide the children's behavior and activities, prevent harm, and ensure safety when Staff A is seen walking through the hallway with a group of children when a 5 yr. old is seen on video leaving the group & entering an unoccupied classroom undetected by Staff A. the child eventually leaves the room & exits the facility where she becomes locked out & is subsequently discovered outside crying by another child care worker. The child was alone for a little over 3 minutes per video footage. |

The center failed to adhere to its child tracking procedure when Staff A reported during a telephone interview that she did not have any child tracking information with her for the children she was supervising when she was responsible for delivering the children to their classroom following arrival at the center via school bus. During this time one child left the group while inside the center. Staff A was unaware the child had walked away from the group until after she was found outside the facility by another staff member. |

Children went without nourishment for longer than 3 hours when lunch was served more than 3 hours after the last nourishment served (breakfast.) For example, in the Panda classroom breakfast was reported to have been served at 9 AM. Lunch was observed being served at 12:20 PM. In the Dino classroom breakfast was reported to have been served between 8:30-8:45 AM. Lunch was observed being served after 12PM. |

Changes to the center menu were not recorded on the copies of the menu kept on file and posted for parents for the lunch that was served. The menu stated pizza, mixed fruit and mixed vegetables would be served. Dominos cheese pizza was served to all classrooms. Beets were served to some classrooms, fresh tomatoes were served to at least one classroom. Fresh fruit was also served. For example, orange slices were served to the Panda classroom, the Dino classroom was served apple slices, the Koala classroom was served pear slices. The fresh fruits and the tomatoes served were not recorded. |

Child #1 was permitted to be in attendance at the center on 9/15/22 without his required Epipen.
In order to prevent children from consuming their allergen the licensee states she relies on a specific allergen notification on food products served. The cook reported she checks ingredient lists however a list of food ingredients to avoid provided by Child #1's allergist was not provided to the cook. Documentation from the food service supplier states the food ingredients should be checked as a means to prevent contact with a potential allergen in their products. |

On 9/22/22 Staff B noted Child #1 to have red spots around his mouth at approximately 10:40 AM and alerted Staff A, the lead classroom teacher. The child's parent was not notified that staff suspected he may be ill. |

Areas of the outdoor play space have been separated by snow fencing and rope. The ropes and snow fencing is sagging in various areas and the snow fencing has separated from the rope in some areas resulting in the snow fencing being on the ground posing a tripping hazard.
A large commercial carpet is on the ground in the play space spanning numerous yards in length. The carpet is not flat & is folded in various areas posing a tripping hazard.

At least 9 inches of mulch is not present under the monkey bars in the outdoor play space. Approximately 1-2 inches of loose mulch is observed under the monkey bars. On 5/13/21 a 5 year old child sustained a broken arm after falling from the monkey bars. |
